For example, a fictional owner’s claim pays eligible treatment but leaves an examination fee unpaid. First locate that fee on the invoice and check whether the relevant benefit was selected. If the invoice description is wrong, the practice can clarify it. If the insurer applied the wrong selected terms, ask the insurer to explain or review that application.
Do not ask a clinic to decide the insurance contract, or ask the regulator to rewrite the clinical record. Separating the factual and contractual questions reduces the chance that each party sends you back to the other without answering anything.
